Policy
Staff will report in writing any incident of vandalism, improper behavior, serious disturbance, violation of the library's rules of behavior (refer to Unruly Conduct in this manual) or incident requiring assistance from the police.
Application
Correctly completed security-related Incident Reports provide written, objective records documenting violations of library behavior. These records may be used to track repeated incidents, to notice violations occurring in various locations, and/or to serve as written records for the police.
Procedure
- Report the incident by using the Incident Report Form as soon as possible after the event.
- Please refer to the detailed instructions for the particulars of how to use the form.
- Be specific and accurate about the date, time, and location of the incident.
- Include a full physical description of the person or persons involved, when applicable.
- Be brief and to the point.
- Avoid judgmental language, slang, and personal comments.
- Please remember: Incident Reports should be considered legal documents; they may be used in court.
- If the form is malfunctioning or inaccessible, the document version of an Incident Report may be submitted. Instructions for sending.
