Accident, injury, or illness involving staff

Policy

Staff will provide immediate first aid to staff in case of accident, injury, or illness. Staff will also report "near miss" incidents that had a high potential for injury to staff or patrons.

Procedure

Follow the same procedures as for a member of the public with these exceptions:

  1. Any accident involving an on-duty employee, no matter how minor, must be reported immediately to the supervisor or person in charge of the section or branch. On-duty volunteers are covered by the County's workers' compensation system and, as with staff, are required to fill an incident report if they injure themselves while on duty.
  2. If an employee or on-duty volunteer sustains an occupational injury or illness that may require medical treatment, follow Multnomah County Administrative Procedure RSK-1. Staff should never drive an employee/volunteer who becomes ill or injured at work to the hospital.  They should be transported by appropriate medical personnel.
  3. If an employee or on duty volunteer is transported to a hospital; contact the EMT PIC (503-201-0522) or other senior manager.
  4. If an employee or on-duty volunteer suffers an occupational injury or illness that does not require medical treatment, follow Multnomah County Administrative Procedure RSK-2.
  5. Accidents (actual or near miss), illness or occupational injury must be reported on the Origami Injured Employee Accident/Incident RSK-2  Incident Report Form.
  6. Workers' Compensation information and forms are available on the Workers' Compensation Program page on Multco Commons.
  7. In all cases, at least the initial report on the applicable form should be sent to County Human Resources within 24 hours of the being reported.  This should be done by the manager or Person in Charge (PIC).  If you have to use the paper form instead of the Origami portal, email a or fax a scanned copy using the information listed on the form.  Also email a scanned copy of the form to lisac@multcolib.org.  You may retain a copy at your work site as well.
  8. Managers/supervisors should review and complete the accident/incident investigation portion of the form; then determine and implement corrective action as needed.  This should be done in conjunction with the impacted employee(s) if there are any.  Lisa Canavan and Risk Management are available for consultation.  A followup report should be submitted once the managers/supervisors portion is complete.
  9. If an employee or volunteer has bare skin exposed to potential blood-borne pathogens or sustains a needle stick, call the CorVel 24/7 Nurse Triage Call Service at 844-296-5159.
  10. All questions regarding Workers' Compensation should be addressed to the Library's Human Resources staff.
  11. Employees may make safety suggestions regarding Workplace Hazards to their supervisor, to a location or Regional Safety Committee member or to a Department/Office Manager verbally or by using the County's RSK-7 form
