New hire employees
Library HR will send in the request. Photos are taken Monday through Friday from 8:00am - 4:00pm at the Multnomah Building 501 SE Hawthorne in room 159. Bring a driver's license or DMV I.D. card when going to have your picture taken. A Passport is also acceptable.
Transfers, LDAS & temporary positions
Library HR will send in the request. However, sometimes appointments change, and that isn't always communicated to Library Facilities to update access to our County ID Cards. We have a form to help us track all these changes. When in doubt, fill it out.
Card stopped working?
Contact Library Facilities (lib-facilities@multco.us) to request a replacement or fill out this form. Facilities will issue a new card using the photo they have on file. Send back the old card when you receive your new one and there is no charge for a replacement. If you think your appearance has greatly changed since the original photo was taken and you'd like a new one, or you'd like to add your pronouns, include that information in the request.
Card lost or stolen?
If you lost your card or it was stolen - Inform Library Facilities staff (Email lib-facilities@multco.us) or call Facilities Dispatch (503.988.3779) to have the old card deactivated for security purposes - remember it's also a key. If you reported the lost/stolen card to Dispatch, contact Library Facilities to request a replacement or fill out this form. The replacement card will be sent to the staff member.
Please don't contact the county's ID section directly to request a replacement badge, an 'approved' requestor must send in the request, so email lib-facilities@multco.us. Once you receive your new or replacement card, it may take a couple of days before the programming starts working. Let us know if your card isn't working more than three days after your receive it.
